The state of Connecticut has identified providers in Connecticut that meet the highest patient care standards for specific procedures and conditions as “Providers of Distinction”. By completing your care with a provider that is tagged as a “Provider of Distinction” in the Find Provider tool, you will receive a cash incentive in the mail. Learn More

As an SPP member, you can access a comprehensive healthcare plan with flexible options to support your health and wellness.

- Low-Cost Office Visits: $15 In-network visit cost, $0 In-network visit cost for preferred providers
- 100% Covered Preventive Care: Stay on top of your health with fully covered preventive services.
- Affordable Prescription Costs: Pay only ($0 for HEP Chronic) $5 or $10 for generic maintenance medications to help you manage long-term conditions affordably.
- Out-of-Network Flexibility: While in-network care maximizes your benefits, you can also see out-of-network providers, with up to 80% of those costs covered.
Plan Information
Eligible dependents generally include:
- Your legally married spouse or civil union partner.
- Your children: Medical and dental coverage through the end of the (calendar) year in which they turn age 26.
- Disabled children may be covered beyond age 26 for medical and dental, with proper documentation from the medical insurance carrier. Contact your HR representative to confirm.
You can make changes to your medical and/or dental plans, add or drop coverage for your eligible family members or enroll if you previously waived coverage during the following times:
- Qualifying Life Event
- If you experience a Qualifying Life Event, you can make changes by contacting your Human Resources office. Be sure to provide the required documentation within 31 days of the event.
- Open Enrollment
- During Open Enrollment, you have the opportunity to review and adjust your health care benefits. It’s a great time to assess your and your family’s needs and select the best plan for you. For assistance, please reach out to your HR staff.
Medical Plan Rates .
If everyone covered by your plan does not complete the age-related Health Enhancement Program screenings and any required chronic condition education by each year, you’ll pay an additional $100 a month for coverage plus you will have an in-network deductible of up to $1,400.
