Diabetes Prevention Program

Did you know over 88 million Americans have Prediabetes — and many don’t even know it? The Digital Diabetes Prevention Program is being offered to state health plan members to help enact healthy habits and lifestyle changes to prevent the onset of Diabetes. The program is run by the same group that administers the Health Enhancement Program (HEP).

This is a 12-month program focused on improving lifestyle behaviors to reduce diabetes risk. The program brings powerful education and motivating support right to your computer, smartphone or tablet.

How the classes work

  • Participants join their live digital classes on the prescribed day and time, with a certified health coach that provides education on preventing diabetes.
  • If you are unable to join the live session, you can make-up the session by watching it on demand, according to your schedule. ​​​​​​​​​​​​​​
  • Individualized coaching support is also available.​​​​​​​
  • Online group challenges.​​​​​​​
  • Tools to help live a healthy lifestyle

How do I Access the Program?

This is an online program that you access though your computer, laptop, or by downloading our App onto your smartphone or tablet. Participants should have access to Wi-Fi, be comfortable using WebEx and navigating online websites.

Who is Eligible?

The Program is available to those individuals who are Prediabetic. There are several ways to determine this. There are blood tests that are part of a preventive exam, such as a fasting glucose or A1c, or you can take the CDC Prediabetes risk test. While this test alone is not always a deciding factor, it is a good indication or your eligibility.

Who is Not Eligible?

If you have Type 1 or Type 2 diabetes, are pregnant, or have a BMI (body mass index) below 25.

Living with diabetes can be challenging. Livongo combines advance technology with Certified Diabetes Coaches to make it easier for you to live the life you want.

The Livongo program offers diabetic state health plan members a free connected glucose meter, testing strips, lancelets and access to coaches to help you monitor and manage your diabetes.

How it works:

  • No more paper logbooks! You get a free glucose meter that will automatically upload your glucose readings to a secure online account and provides real-time personalized tips.
  • Reorder glucose strips and lancets right from the meter. They are mailed to your home at no cost to you. When you run out, more will be shipped. FREE!
  • Real-time support when glucose levels are out of range and you can opt to have updates sent to your provider to keep them informed.
  • Communicate with a Certified Diabetes Coach anytime about diabetes questions on nutrition or lifestyle changes.

Glucoze Zone

GlucoseZone/Fitscript is an online fitness and coaching program for getting active, losing weight, and lowering your A1c.

With online workouts and coaching that can be done at home or outside, GlucoseZone/Fitscript is here to provide you with the exercise and support you need to have success with your diabetes prevention and management.

The program is available to all HEP participants. You don’t have to be diabetic to enjoy the lifestyle benefits of using GlucoseZone/Fitscript.

  1. Go to app
  2. Select “yes I do” when asked if you have a code
  3. Select ‘Connecticut HEP’
  4. Insert the code: CTHEP
  5. Register your name and email.

Health Enhancement Program (HEP)

The Health Enhancement Program (HEP) rewards you for keeping up with your preventive health visits and chronic disease management. For those with diabetes, if you have questions about your medications or need help managing your condition, a dedicated clinical staff is available to help – just call the HEP customer care team at (877) 687-1448 and ask to speak to a nurse.
