Medical Weight Management

As of July 1, 2023, medications prescribed for weight loss or weight management are only covered if the prescribing provider is part of one of the health plan’s Medical Weight Management Programs.

VIRTUAL WEIGHT MANAGEMENT

FlyteHealth is a virtual medical weight management program that combines medication with lifestyle behavior to help participants improve their health. In addition to virtual check-ins with a medical doctor, a registered dietician and nurse practitioner are available to answer questions and provide support along the way.

Starting July 1, 2025, all state Medical Weight Management participants must pay a $12.50 monthly program fee. This fee is to be collected via bank debit or credit card. The same bank card will be used for any applicable no-show or late cancellation fees. Please look at the FAQ document below for details.

If eligible, you will:
  • Work with a doctor who creates a personalized treatment plan based on your health history, genetics, lifestyle, and readiness to change. This plan may include new or adjusted medications, meal planning, goal setting, and gradual activity increases. You’ll also complete lab tests and a Health Risk Assessment on the FlyteHealth Patient app.
  • Have regular check-ins with your dietitian and nurse practitioner will help monitor your progress, address challenges, and make adjustments as needed. Your care team will stay connected with you through telehealth visits and app communication, providing resources like articles and courses to support your journey.

  1. Apply to see if you’re eligible at joinflyte.com/care-compass. Click the Get Started button and fill out the application form. If you are deemed eligible, you will be automatically enrolled in the FlyteHealth program. If you are not eligible, you will receive an email or phone call with further instructions.
  2. Download the FlyteHealth patient app. You can use the app to log meals, exercise, and your weight. Plus, it has a library of helpful information on losing and maintaining weight.
  3. Complete the checklist in the app. This checklist helps your FlyteHealth care team learn more about you and your health. Lab work and answering the Health Risk Questionnaires are required.

Hartford Healthcare

Managing weight can be challenging, whether due to genetics, health conditions, or a busy lifestyle. Hartford HealthCare’s medical weight management program combines specialists in weight management, behavioral health, nutrition, and exercise to create personalized plans tailored to individual needs. The program provides structured guidance and support to help participants reach their weight goals and build a healthier lifestyle.

Registration Process
  1. Visit joinflyte.com/care-compass.
  2. Click the Get Started button to fill out the application form.
  3. Select Hartford HealthCare’s Medical Weight Loss program.
  4. Complete the checklist in the app, which includes consent forms, Health Risk Assessment (HRA), and lab (blood) work. This checklist helps your care team learn more about you and your health.
  5. A Hartford Healthcare office will call you to set up your first in-person appointment once the checklist is complete. If you need to contact the Medical Weight Loss team, call us at 1-855-792-6258.

A personalized treatment plan will be recommended, which may include FDA-approved medications and support from registered dietitians and behavioral health specialists. The program is designed to help you find the approach that best fits your health and lifestyle. Medication is just one part of the program. Additional support includes:

  1. Nutrition guidance: A registered dietitian will help you follow one of several doctor-recommended meal plans.
  2. Exercise planning: Fitness professionals will create exercise plans tailored to your needs.
  3. Behavioral health support: Specialists help address emotional challenges and remove barriers to success.

Children’s weight management

Connecticut Children’s is partnering with the state of Connecticut health plan to offer a weight management program tailored to children, teens, and their families. These programs are designed
to suit the specific needs of each child, considering factors such as age, BMI, and family requirements. Medication coverage varies and may not be part of the program.

The Pediatric Obesity Center at Connecticut Children’s offers behavior-based weight management programs for children and adolescents. Their specialists help children and their families change behaviors to establish healthier physical and emotional lifestyles. Their comprehensive, 12-month clinical program includes regular visits with the care team, including a medical doctor, physical therapist, psychologist, and dietician. Telemedicine visits are available for some care team visits.

The following programs are offered:
  • Fit 5 Tykes (ages 3-5): Tailored to address the specific needs of families and their children.
  • Fit 5 T.E.A.M. Club (age 6-9)
  • Fit 5 Teens and Tweens (ages 10-12 and 13-18).
  • Individualized Fit5 program and services for youth who need to lose weight to undergo a medical procedure.

BMI > 95th percentile or BMI 25-30. for all children 3-18 years. Height and weight measurements are used to calculate BMI. BMI percentile is based on age and sex-specific CDC growth charts or severe obesity growth charts.

Contact the Connecticut Children’s Weight Management Coordinator by phone or email. They will request lab work and information from your pediatrician. After receiving all the necessary information, an appointment will be scheduled at the Screening Clinic. During this visit, a pediatrician will review your child’s lab work and address any medical questions. A pediatric psychologist will also meet with you to discuss available weight management strategies.
