Voluntary Term Life Insurance
Voluntary Life Insurance provides coverage up to $500,000 for active employees. Premium rates vary, based on the coverage elected and age group in which the covered individual is positioned. If the insured passes away during the policy term, the death benefit is payable to the designated beneficiary.
Eligibility
Active State Employees are eligible for Voluntary Life Insurance if they work at least 17.5 hours per week and are enrolled in Basic Group Life Insurance.
How to Enroll
- Contact Unum Life Insurance Company’s enrollment representatives at 833-703-1967 and note employer code 1357722. Please note: Colonial Life manages the telephonic enrollment center for Unum, which is why their name is mentioned in the HIPAA statement when you call.
- To register for the online portal, enter your First and Last Name, the company identifier as “State of CT,” the last 4 digits of your Social Security Number (SSN), and your Date of Birth.
Guaranteed Coverage
New employees, under age 60, who enroll within the first 6 months of employment can obtain guaranteed coverage up to $100,000 and up to $30,000 for their spouse. Employees enrolling after this initial period are subject to evidence of insurability requirements.
Current Policyholders
For policyholders wanting to view their Unum Voluntary Life elections, find an election form, or access other benefit documents, log in to the portal and follow the prompts. Note: If you forget your login credentials, use the “Forgot Username?” or “Forgot Password” on the Employee Navigator log-in screen.
