Prevent Diabetes

Did you know over 88 million Americans have prediabetes — and many don’t even know it? The Digital Diabetes Prevention Program is now available to state health plan members, aiming to help develop healthy habits and lifestyle changes to prevent the onset of diabetes. This program is administered by Wellspark.

This 12-month program focuses on improving lifestyle behaviors to reduce diabetes risk. You’ll receive powerful education and motivating support directly on your computer, smartphone, or tablet.

  • Participants join their live digital classes on the scheduled day and time, guided by a certified health coach who provides education on preventing diabetes.
  • If you’re unable to attend the live session, you can watch it on demand at your convenience.
  • Individualized coaching support is also available to cater to your specific needs.
  • There are online group challenges and various tools to help you live a healthy lifestyle.

The Program is available to individuals who are prediabetic. You can determine your eligibility through several methods:
  • Blood tests that are part of a preventive exam, such as fasting glucose or A1c.
  • The CDC PREDIABETES RISK TEST, which provides a good indication of eligibility, though it is not always a deciding factor.
You are not eligible if you:
  • Have Type 1 or Type 2 diabetes
  • Are pregnant
  • Have a BMI (body mass index) below 25.
Past participants are welcome to re-enroll in this program.

  • This is an online program that you access though your computer, laptop, or by downloading our App onto your smartphone or tablet. Participants should have access to Wi-Fi, be comfortable using WebEx and navigating online websites.

Manage Diabetes

If you have Type 1 or Type 2 diabetes, you can receive support with access to a diabetes health coach, free testing supplies and tips to manage your A1c.

State health plan members ages 18 and older are eligible for the Management Program if they have either Type 1 or Type 2 diabetes.

I’m grateful for this opportunity to participate in Virta to help manage my diabetes. This is a lifestyle change and very rewarding to my health. Reverse Diabetes

Virta makes Type 2 diabetes reversal possible. This virtual clinic can help you lose weight, lower your blood sugar, and reduce medications. Members learn to eat their way to better health with personalized nutrition plans and support from medical providers, professional coaches, and digital health tools. No medication, surgery, or calorie counting is required to see results.

State health plan members aged 18 and older. The Reversal Program is exclusively for those diagnosed with type 2 diabetes.
