Providers of Distinction

The state of Connecticut has identified providers in Connecticut that meet the highest patient care standards for specific procedures and conditions as “Providers of Distinction”. By completing your care with a designated “Provider of Distinction”, you will receive an incentive*.

Effective July 1, 2026, some providers will drop from the Providers of Distinction program. To earn an incentive for an eligible procedure from July 1, 2026- June 30, 2027, your provider must have the “Earn an Incentive” flag in the Provider Search. Incentives earned from July 1, 2025, to June 30, 2026, will no longer be redeemable after December 31, 2026.

How it Works

Use the Find Provider tool to locate Connecticut-based providers with the “Earn an Incentive” flag. These providers are designated as Providers of Distinction for the care and procedures listed in the table below.

Simply receive eligible care from a Provider of Distinction. Almost 500 providers have been designated as Providers of Distinction, spanning the state, including onsite and virtual back and spine pain management programs.

When you complete an eligible procedure with a Provider of Distinction, you will receive a letter and email notification once your reward is available to claim. This can take up to 60 days after your procedure, once insurance processing is complete.

To ensure you receive notifications of earned rewards, it is important to log into your Quantum Health benefits portal and use the Provider Search. Also, updating your address with your Human Resources person is helpful for all mailings.

To earn an incentive, the provider you choose must be listed as a “Provider of Distinction” in the Find Provider tool.

  • *Incentive rewarded once per pregnancy. Members who have any pregnancy-related visit with a designated Provider of Distinction between 7/1/2026 and 6/30/2027 will be eligible to receive a $250 gift card. To qualify for this incentive, members must receive prenatal care from a Provider of Distinction. Prenatal care is identified using procedure codes for qualifying visits within 275 days prior to delivery. Members who use a Provider of Distinction only for the delivery, without qualifying prenatal visits, are not eligible for this incentive.
  • **Spine care incentives are awarded once in a lifetime. The $100 spine care incentive is awarded once for a visit with a spine specialist or for completing the Spine Pain Management evaluation. The $200 incentive for the spine pain management conservative treatment series is also a one-time lifetime maximum.
  • ***The $200 Conservative Treatment Series incentive is awarded only once in a lifetime per person.

Visit a “Provider of Distinction” specializing in back and spine care to receive the $100 incentive for the assessment. The spine care incentives are only available once per lifetime coverage. To locate a low back spine care specialist in the Find Provider tool, insert the word “Spine” in the search box.

By completing the clinical spine evaluation at a listed location, you will be eligible to earn the $100 incentive. If you also complete the conservative treatment series (i.e., physical therapy, occupational therapy, acupuncture, etc.) with the same provider, you will be eligible for a $200 incentive. **Spine Care incentives are awarded once in a lifetime.
